What kind of party are you interested in? *

A "station" seats one guest. The number of Stations you select above includes the guest of honor (the birthday boy/girl). Aside from the gaming stations, your party gets at least one table reserved for your group for parents and other guests to use during the party. If you need more or less stations or need the party to last for longer or shorter amount of time, we can setup custom options to fit your group's needs. Once you submit this form, staff at your local store will contact you to arrange the details.

When would you like to have your event? *

A peak fee may be applied if your party takes place during exceptionally busy times. These times vary per store, but we'll go over everything when we call to confirm your reservation.

I, customer, understand that the deposit paid is non-refundable and the total cost of the deposit will go towards the total balance. I also understand that if the date or time of my scheduled event needs to be adjusted that I may do so until one week (7 days) prior to the scheduled date. I understand that I will be ultimately responsible for the guests attending should any damage occur to either PLAYlive property or guests.
